MAJOR DUTIES & RESPONSIBILITIESSpecificCase Management / Psychosocial Care * Staff will provide psychosocial care to patient and their significant others in Singapore's multi-cultural context as directed by the Medical Social Worker
Will review intervention plans and work with the management of the case together with the Medical Social Worker.
They will conduct independent assessment of straight forward and non-straight forward financial cases and apply for relevant national financial schemes for patient and their family members
Prepare brief social assessment for the Medical Social Worker for continuation of follow up
Conduct home visits and organizations engagement independently and provide assessment of the home situations or organizations to the Medical Social Worker
Staff will need to meet department clinical indicators.
Member of Quality improvement related projects that will improve patient's outcome.
Group Work * Assist in logistics support and co-facilitate group work sessions for patients/ significant others as directed by the Medical Social WorkerCommunity Work * Contribute in community mapping and ground engagement as directed by the Medical Social Worker
Plan and work with the social work coordinator to plan for community events
Education / Clinical Supervision * Participate as a member of Department's education or orientation team.General * To apply and adhere to social work's professional ethics and values
To be actively involved in Department's initiatives
Any other duties as assigned by the team leader or Supervisor and Head of Department
JOB REQUIREMENTSEducation / Training / Experience * Minimum GCE 'O'/'A' Levels/ NITEC in Community and Social Services
Candidates with 2 years of relevant social service will be an advantage.
