OverviewThe Position Holder will be responsible for the strategic engagement of agency partners to augment NUHS' role as the population health manager for the western region. This includes ideation and management of collaborations with partners and within NUHS to support the roll out of new policies, advancing social-health integration and enhance the delivery of care in the community.Job ResponsibilitiesYou will be responsible for the following:
  • Regular engagement with agency/community partners and internal stakeholders on policy rollouts (e.g. HealthierSG) and to improve care integration in the community
  • Managing feedback and issues with/from partners
  • Keeping abreast of developments within social and community care organisations
  • Supporting team in development, implementation, data collation, evaluation and reporting of initiatives and collaborations within NUHS and with agency partners or social care providers
  • Meeting and event organisation
  • Taking minutes for meetings and events and following up on issues raised
  • Supporting implementation of NUHS Population Health strategies and regional health plan
Requirements
  • Good Degree, preferably in Life Sciences, Business Administration or related qualification
  • 1 - 2 years of working experience, prior experience in community care or strategic partnerships will be an advantage
  • Experienced in project management and working with multiple stakeholders
  • Good communication, interpersonal and organisational skills
  • Ability to multi-task and work independently
  • Ability to work in a fluid environment amidst multiple demands
