- Evaluate, design, deliver and monitor client care needs
- Oversee and guide a team of Community Caregivers
- Create, execute and revise individualise client care plans
- Provide support and educate client
- Arrange client for additional care or services as needed
- Perform home visitation for client
- Assist with administration work and reports
- Oversees resources for nursing operation
- Support the execution of occasional events, which may take place on weekends and public holidays.
- Registered with Singapore Nursing Board
- Diploma/ Degree in Nursing
- At least 1 years of nursing experience
- 5 days work week (Shift: 8.30am to 5.30pm or 9.00am to 6.00pm)
