NilKey Accountabilities Patient Care • Participates in the planning, delivery and evaluation of patient care within the haemodialysis clinical setting: − Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluate effectiveness of care − Adheres to all FMC Policies and Work Instructions related to clinical and operation management − Reviews blood test results and report to Medical Director of any abnormalities − Educates patients on their diet, fluid, medications and blood pressure management to optimise their clinical outcomes − Motivates and encourages patient’s adjustment to CHD − Monitors the water treatment system and fluid for dialysis to ensure provision of safe, quality dialysis therapies • Establish an effective relationship with patients and maintain good communication with patients and relatives; and give adequate support. • Ensures/promotes a safe, secure and healthy working environment to staff and a comfortable, safe environment for visitors and patients as detailed in the IPC and environment policies • Ensure all equipment is clean, safe and in good working condition; and report defects through appropriate channels. • Maintains accurate patient information in medical records, including Therapy Data Management System (TDMS)/EuClid Clinical Information system and abides by confidentiality and Privacy Policy. • Protects the rights of patients by adhering to Privacy Principles and providing an excellent standard of care • Actively participates in the quality improvement process by reporting all incidents, hazards and opportunities for improvement

  • Assume responsibility for own professional competence by demonstrates willingness to evaluate own nursing practice – include complete of annual clinical competencies assessment.
  • Participate in continuing education programs to gain current knowledge of development. • Use staff ‘performance review’ constructively for personal and professional development.
  • Maintain a professional image among colleagues and public. • Comply with required dress code. Qualifications and Experience
  • Diploma / Degree in Nursing.
  • Registration with Singapore Nursing Board (SNB)
  • Min 1 year as Registered Nurse in Singapore – For foreign applicant
  • Fresh Diploma graduate is welcome
  • Advanced training in hemodialysis is a plus
  • Certified and valid Basic Cardiac Life Support (BCLS).
Requirements
  • Understand Quality Improvement process and Risk Management obligations.
  • Ability to continuously reflect on nursing practice and apply learning as appropriate.
  • Dedicated and committed to deliver excellent patient care
  • Effective communication and interpersonal skills.
− Good command of English and a local language (preferred)
  • Able to give and receive comprehensive nursing reports.
  • Computer literate with knowledge of MS-Office for words and excel is an advantage.
