
Community Care Manager, Daycare

- Case Management & Administration
- Complete the Referral/Screening process and accept/reject referrals.
- Coordinate and co-conduct Initial Assessments, including documenting information on admission suitability and care planning that fulfil the ministry service requirement
- Develop and coordinate the delivery of Discharge plans, including referrals to other providers.
- Provide input on process improvement activities
- Care Planning
- Coordinate a team to support, bi-annually review and update Individual Care Plans (ICPs)
- Lead the development, communication and review of non-clinical aspects of ICPs
- Input into process improvement activities for ICP development and evaluation.
- Care Delivery
- Ensure quality of care from the care support team by adhering to Standard Operating Procedures (SOPs).
- Investigate and complete documentation for relevant incidents and escalate complex issues to the Cluster Manager timely
- Support the collaboration with external agencies, including requests for new services.
- Create a supportive and inclusive environment that promotes active social engagement and participation amongst the senior and care staff
- Conduct periodic programme audits and evaluations
- Stakeholder Engagement
- Facilitate and complete the intake, admission and discharge processes with Next of Kins(NOKs) or Caregivers.
- Co-conduct post-admission onboarding of Seniors with the clinical team.
- Update NOK / Caregiver on changes to ICPs biannual review or changes in clients‘ condition
- Address and escalate complaints and feedback from NOKs, Caregivers or Seniors.
- Support delivery of events and engagement with stakeholders.
- Support Medical Social Worker with Advance Care Plan (ACP) conversations.
- Administration & Operations
- Be accountable for the centre operating expenses and ensure that utilisation is kept within the centre's budget.
- Be accountable for the utilisation and occupancy targets.
- Evaluation of vendor's products or services, including managing trialling of and recommendations for new technology or equipment.
- Liaise with the town council, building and facilities management on maintenance of centres.
- Cascade crisis response SOPs to staff and report to HQ.
- Coordinate transportation requests for seniors.
- Review and approve roster and leave requests for the Care Team.
- Quality Management
- Monitor staff compliance with Infection Prevention & Control (IPC) guidelines & standards
- Prepare and support Internal and External Audits and implement improvement areas.
- Cascade and monitor compliance to non-clinical SOPs.
- Evaluate the effectiveness of non-clinical SOPs and provide feedback to the Quality Management team.
- People Management
- Lead the team to build up a credible professional image and positive branding of NTUC Health
- Provide feedback to the planning, recruitment and evaluation of Volunteer Management.
- Accountable for workforce planning and recruitment of staff.
- Monitor and ensure staff training needs are met.
- Support communication and monitoring of staff performance and compliance to Company policies.
- Adopt a collaborative approach towards working with internal and external stakeholders.
- Possess change management experience and be an effective communicator.
- Be proficient in Google Suite Applications and digital tools (i.e. WhatsApp, YouTube, Zoom).
- Have a strong sense of adaptability (i.e. being creative and finding new ways of getting things done).
- Have a caring, empathetic and patient personality to establish positive relationships with seniors and caregivers.
- Possess the ability to lead people and manage clients from diverse backgrounds and levels.
- Be proactive and observant in identifying and attending to the needs of seniors and staff.
- Possess a growth mindset and embrace continuous learning and development.
- Degree holder or equivalent
- Possess a minimum of 5 years of experience in managing and leading teams.
