Job Description:The Social Work Associate works in the field of Social Work and is trained in psychosocial assessment and intervention. He/She supports the coordination of casework, group work, and psychosocial programmes, provides assistance in admission and discharge planning, and helps families navigate financial assistance schemes. The Social Work Associate also assists in administrative functions to support the social work team.Admission and Discharge Administration, Including Financial Counselling and Means TestingManage the admission and discharge process for seniors, including pre-admission work such as handling new referrals, liaising with external agencies, and coordinating with internal teams to ensure a smooth transition into and out of the nursing home.Conduct financial assessments and means testing to determine eligibility for financial assistance schemes (e.g., Medifund, subsidies, or other funding sources).Provide financial counselling to seniors and their families, explaining funding options and addressing concerns about care affordability.Assist in setting up and maintaining administrative matters related to patient needs, from onboarding to discharge.Liaise with healthcare teams, families, and external agencies to coordinate applications and ensure timely approvals.Case Management Support for Seniors and Their FamiliesAssist in case intake and assessments, gathering relevant information on seniors' BioPsychoSocial functioning, including mental well-being and financial status.Provide case management support by monitoring care plans, tracking progress, and following up on interventions to ensure continuity of care.Work closely with team members in developing Individual Care Plans (ICPs) to meet seniors' specific needs.Work closely with Medical Social Workers (MSWs) in addressing complex cases, including those involving mental health concerns or family conflicts.Collaborate with healthcare professionals and external agencies (e.g., hospitals, family service centers) to ensure holistic support for seniorsResolve client feedback and concerns, ensuring that client concerns are addressed in a timely and appropriate manner.Provide accurate and professional communication to clients, families, and business partners regarding policies, procedures, and available services.Handle client concerns with clarity and professionalism, ensuring appropriate resolution and maintaining a positive stakeholder relationship.Support the Formation and Implementation of Psychosocial ProgramsPlan, coordinate, and execute psychosocial programs that enhance seniors' and caregivers' well-being.Engage seniors in programs that promote psychosocial wellness and community integration.Collect and analyze feedback (data collection and entry) from seniors and families to evaluate program effectiveness and recommend improvements.Administrative Support for the Social Work TeamMaintain accurate documentation of case notes, financial assessments, and intervention records in compliance with PDPA and organizational policies.Prepare reports, referrals, and funding applications for review by MSWs and management.Handle administrative tasks such as scheduling appointments, coordinating meetings, and managing databases for case tracking.Support the team in liaising with external agencies for grants, donations, or sponsorship opportunities that benefit residents.Ensure adherence to MOH service requirements regarding financial and quality standards through regular internal audits.Assist with logistics and operational support when required.Any other related duties as directed by the reporting officers.Requirements
  • Minimum Diploma in Healthcare or related Social Work fields.
  • Experience in the nursing home and social services sector is beneficial but not required - candidates with relevant skills and passion are welcome to apply.
  • Good time management skills, well-organized, and efficient.
  • Takes initiative and able to work well independently and as a team player.
