- Plan, implement and monitor the outreach, care process, referral and follow-up workflow.
- Provide guidance and supervision to the team to provide care and support, in alignment with the overall CREST programme objectives, adhering to the relevant ethical and legal guidelines.
- Provide holistic and integrated care for clients and caregivers through basic emotional support, monitoring, service linkage and conducting meaningful activities.
- Establish and review the workflow processes including identification, referral and escalation work processes.
- Responsible for establishing the network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers. To advocate and collaborate with other partners to facilitate preventive activities and increase awareness of building Dementia-Friendly Community, as part of supporting the broader Dementia Friendly Singapore movement.
- In charge of the analysis and review of the collated data, and monitoring of the team's progress and performance.
- Responsible for ensuring timely and accurate submissions of the claims and progress report to AIC.
- Ensure programme data and documents are kept and maintained, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
- Oversee the training and professional development of the CREST staff team with relevant competencies to provide the appropriate care and support for clients and caregivers, aligning to the intended care model of CREST. This includes ensuring all new/existing staff are clear on their job scope and responsibilities.
- Ensure that the team practices are in adherence to the prevailing organisational, ethical, safety and legal requirements, where relevant.
- Bachelor's Degree preferably in Social Work / Psychology / Sociology / Gerontology / Counselling
- Minimum 3 to 5 years' direct work experience with clients with mental health issues and/or dementia, preferably with experience in programme management
- Good knowledge of the geriatrics and community services
- Proficiency in Google Suite applications
- Good communication skills (oral and written)
- Passion in community services and service oriented
- Good judgement and decision making skills
- Good in problem solving
- Good record keeping abilities and attention for details
- Possess 1-2 years of experience in administration
