This is done with a team of trained social workers using a case management approach, monitoring and counselling services. As a Social Worker, you will be required to carry out casework and apply intervention strategies appropriately to improve the well-being of the client and support the families and caregivers in the community.You will need to

Responsible for managing case referrals to support clients' needs
Conduct assessment, planning and intervention for cases by engaging client and building rapport, performing needs assessment and advocating for clients' needs
Develop and review appropriate intervention strategies in consultation
Refer and coordinate care services for clients
Provide information on the various assistance schemes or care options available
Raising awareness and facilitating advance care plan for seniors
Conduct outreach activities to the community

You should

Thrive in a collaborative and dynamic environment
Have strong communication and stakeholder management skills
Be proficient in Google Suite Applications

It would be great if you

Have experience in Social Work
Are able to converse in local languages

Qualifications:

Bachelor's Degree preferably in Social Work
Minimum 3 years' experience, preferably in community or healthcare settings in Singapore
