Job Description:
  • Act as a coordinator for the admission of residents under Ministry of Health (MOH) Portable Subsidy
  • Perform financial counselling for all residents under MOH’s Portable Subsidy, including the preparation of estimated bill size, admission contract and other administrative matters
  • To perform and monitor National Means Testing (NMTS) for portable subsidy residents and ensure the resident NMTS are valid and updated according to MOH’s requirement
  • Refer referrals and new admissions assigned by AIC for all ECON Medicare Centres and Nursing Homes
  • Monitor and track the available licensed beds for portable subsidy residents all ECON Medicare Centres and Nursing Homes
  • Act as a resource person for the MOH ILTC portal and IRMs for all ECON Medicare Centres and Nursing Homes
  • Raise referrals and applications for Nursing Home subsidy for ECON private resident
  • Assist Home and Centre in putting up the following applications for their residents
  • Shelter Home
  • Day Care
  • Day Rehab
  • Home Modification
  • Senior Mobility Fund
  • Meals on Wheelsg.
  • Any other Home and Centre Based schemes available
  • Supervise and check all applications for government schemes prior to submission. Applications would include the following, and other schemes that may be available
  • Interim Disability Assistance Programme for the Elderly (IDAPE)
  • Eldershield Claim
  • Medical Fee Exemption Card (MFEC)
  • Medifund Claime. Elderfund
  • Careshield Life / Medishield Life
  • Home Caregiver Grant
  • PGDAS
  • Silver support schemes encashment
  • Any other financial schemes available
  • Supervise in the monthly tracking of Subsidy Change report for billing and accuracy forall ECON Nursing Homes and Medicare Centers
  • Assess and put-up applications for medifund claims quarterly for all ECON NursingHomes and Medicare Centers
  • Assist the Social worker or supervisor in engaging with clients for psychosocial needs,conflict management and family conference
  • Perform such other duties and responsibilities that may be assigned from time to time
Requirements:
  • Diploma in Social services / Nitec in Community or Social Services or any equivalent qualification with at least 3-year relevant working experience
  • Good interpersonal and communication skills
  • Team player
  • Service oriented and customer-focused
  • Competent in IT skills
